The Cider House Rules is John Irving’s expansive, morally complex novel set primarily in Maine in the first half of the twentieth century. At the remote orphanage of St. Cloud’s, Dr. Wilbur Larch—obstetrician, ether addict, founder of the orphanage and clandestine abortionist—delivers unwanted babies, finds homes for some of them, and performs illegal abortions for women who have nowhere else to turn. His favorite orphan, Homer Wells, repeatedly fails at adoption and ultimately remains at St. Cloud’s, where Larch trains him in obstetrics and hopes he will inherit both his medical skills and his mission.

As Homer grows up and eventually leaves the orphanage for work at an apple orchard and cider house, he is drawn into a wider world of love, friendship, class divisions, and wartime upheaval. The novel follows his coming of age and his struggle to reconcile gratitude and loyalty to his mentor with his own opposition to abortion. Through intersecting lives and generations, Irving explores reproductive rights, institutional cruelty toward women and children, the meaning of family, and the difficulty of making ethical choices in an unjust society.
